This fake wasn't created by pixel painting body parts onto a picture of Sarah Michelle Gellar, it was done by making a portion of someone else's nude body semi-transparent and overlaying that picture onto the original picture of Sarah. The end result makes it appear that you can see though Sarah's blouse. It's becoming a very popular form of fakery.
Below is another illustration that should make the process more clear. This graphic shows the entire picture of the nude model made semi-transparent and then laid partially over the original picture of SMG. If only the section of the nude photograph that exactly aligned with SMG's blouse was used instead, you would have what appears in Case #240a.
